Advanced Care Practitioner - Paramedicine

Accountable to North Care Network Clinical Director

Location Travel between the sites across the PCN

North Care Network, the primary care network including the surgeries at:

Central Surgery – Barton

Winterton Medical Practice – Winterton

Bridge Street - Brigg

This role does include working across the week and weekend at all three practice sites, including rotas to cover evening working up to 8pm.

The salary for this role is from £48,500 to 50,700 dependant on your experience with 37.5 hours per week available. This is a permanent role to commence ideally April 2023.

North Care Network has 3 active likeminded surgeries based in North Lincolnshire, with a registered patient population of 33,533. The network has experience of working with ACPs, offering support and training where required.

We are looking to expand our multi-disciplinary team by recruiting an additional Advanced Care Practitioner with Paramedicine.

Working across the PCN you will act within your professional boundaries to provide care for the presenting patient, delivering a high standard of advanced autonomous clinical skills with in-depth theoretical knowledge and evidence-based. You will become an important and valued member of our existing team.

This role involves working with urgent care and on the day presentation to primary care, patients with chronic disease and home visits to patients for assessment as well as working in care homes to support the care home contract.

You will be a registered practitioner looking to move into or already working in a community focused role.

On a day-to-day basis, you will take both a clinical leadership role, solving complex and more challenging diagnosis and provide supervision to support growth, enhance services and reduce risk. In this role, you will build capacity by successfully influencing, challenging, and leading. Your approach alongside your knowledge, skills and experience will make a tangible and quantifiable difference across the PCN.
