The post holder will assist in the assessment and supportive interventions for young people who are assessed as needing a tier 4 service but can safely be managed in their own home with intensive input, as part of their care plan, in some cases working with young people who present in crisis or in an acute mental state and are considered high risk.

1.1 To work as part of a multi-disciplinary team ensuring good communication with other team members, giving and receiving support and dealing with any issues that inhibit excellence in client care that occur with colleagues

1.2 To have a particular close working relationship with the care co-ordinators of clients with whom the post holder is working.

1.3 To have a particular working relationship with the Home treatment Team Practitioners and support them in implementing care plans.

1.4 To regularly meet with supervisor and manager to ensure high quality practice and that the post holder is supported and therefore able to carry out the demands of the post.

  • To participate in the assessment of young people’s needs in conjunction with qualified staff, with a particular responsibility to assess young people’s needs in relation to their capacity to maintain independent living
  • To assist the young people in maintaining that independence
  • To monitor young people for changes in mental and physical state and compliance/concordance with treatment, reporting back promptly to qualified staff.
  • To support and facilitate young people in the Recovery process
  • To participate in groups run by the team as required
  • To work within agreed care plans and risk plans
  • To have or attain a good knowledge of the Recovery model
  • To assist in the planning and development of programmes of care
  • To provide planned community care to young people in various settings, including emotional support and assistance with activities of daily living
