A full time and part time Social Worker is required in the Preparing for Adulthood Team based at the Council House, Derby.

The successful applicant will be based within Derby City’s Adult Social Care ‘Preparing for Adulthood Team’, working with young people aged between 18 to 25 transitioning from Children’s to Adult Social Care services. The team accepting referrals from age of 14, to enable to build good working relationship with young people and have a great understanding of their needs.

The team works with young people who have Education, Health and Care Plans and Care Act eligibility, often having complex levels of disability, health and/or social care need which affects their daily lives and independence.

The Preparing for Adulthood Team works proactively and collaboratively with schools, colleges, education, health, housing and children's services, to support the smooth transition of young people into adulthood. This includes working alongside the SEND team and contributing to the young person’s Education, Health and Care Plan.
The successful applicant will work collaboratively with families and young people to assess needs and eligibility and develop personalised support plans which deliver social inclusion, independence and good outcomes for our customers.

We are looking for candidates with the ability and drive to motivate, encourage and support young people to be the best they can be. They will need to be an excellent communicator, with the ability to develop good working relationships and engagement with young people with a wide range of disabilities and communication needs, their carers and allied professionals.

The successful applicant will require the necessary skills, knowledge and experience to undertake assessments, reviews, support planning, mental capacity assessments, best interests meetings and to undertake safeguarding investigations in a range of settings. This work may involve attending court when required. They will have case responsibility and carry a case load and also work jointly with other team members. They will be required to strengthen and support the quality of service delivery in the team, contribute to peer group meetings, be open to case discussion and have regular supervision.
We are looking for candidates with assessment and support planning skills, experience of complex casework with people with a range of disabilities and health needs, a commitment to person-centred practice and knowledge of adult social care legislation which underpins this role.

Experience of working with young people with a learning disability and /or autism would and working with young people in transition from Children Services to Adult Care be an advantage.

All employees will normally be appointed to the minimum of the grade for the job. For reasons connected with a particular need of the Council it may be appropriate, subject to approval, to appoint to a higher point in the pay grade.
