The role

Care assistants help people to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible.

You will visit Service Users in their own homes and provide appropriate care and support as defined in the Care Plan. This will include aspects of personal care, administering medication, monitoring of vital signs, exercise, and activity

The aim is to rehabilitate , to support and enable Service Users in a range of activities to promote independent living and monitor their well-being.

Shifts patterns are between 7am-2pm and 4pm-10pm working alternate weekends. You will visit service users within the Welwyn Garden City and Hatfield surrounding areas.

• Flexible contracts available from 20 to 40 hours a week

We strive to provide care the best care possible for service users within their own homes to help them remain as independent as possible. Within our Reablement Pathway you will be supporting a range of service users including those regain their independence after returning home from hospital supporting in the Welwyn Garden City and Hatfield surrounding areas.
